Repeat Prescriptions

This may be requested by using your repeat prescription slip.  Please tick items required and either hand in your slip to the surgery or post it with a stamp addressed envelope, if you wish it to be returned to you by post, or let us know if you want it collected by a local chemist. 

Please give 48 hours notice for repeat prescriptions and allow extra time if you send by post or at weekends or holiday periods.

Please allow 48 hours, excluding weekends and Bank Holidays, for your request to be processed. Any problems please telephone the surgery.
